Bwaila District Hospital

Bwaila Hospital, still widely known as Bottom Hospital, is a district hospital in Lilongwe's Old Town, close to the central market and just south of the M1. It sits roughly 4 km from Kamuzu Central Hospital, to which it refers its highest-risk obstetric cases. Maternity is its defining service. The hospital recorded around 14,000 deliveries a year in 2014; reporting in October 2025 put it at 68 to 75 births a day, and by August 2026 at 115 to 120 a day, made up of roughly 90 normal deliveries and 25 to 30 Caesarean sections, with about 168 emergency obstetric operations a week. A 2018 report recorded 220 beds and 300 admissions daily. It is staffed mainly by midwives, nurses, clinical officers and non-specialist doctors rather than consultants. The Bwaila Fistula Care Centre is the only facility in Malawi performing obstetric fistula repair surgery. It has 45 beds, a dedicated operating theatre and 14 specialised nurses, treats around 400 women a year at a repair success rate above 90 per cent, runs repair camps of about 90 women at a time, and trains former patients as community ambassadors. It operates as a collaboration between the Government of Malawi, the Freedom from Fistula Foundation, UNFPA and UNC Project-Malawi. The RuSH clinic for reproductive and sexual health opened in November 2024 and treats 80 to 100 patients a day. The labour ward offers 24-hour HIV counselling and testing under the prevention of mother-to-child transmission programme. Maternity services are free. The original Bottom Hospital, built under British colonial rule for the African population, had a single labour room; its replacement introduced individual private delivery rooms.
