Institutions must completely fill out the "Form for Registration of Institution" and submit it to the Director General at TEVETA to be registered to offer TEVET training (Technical Education, Vocational and Entrepreneurship Training Authority). This form must be filled out in block letters and submitted with the supporting paperwork requested on the page. Any missing required documents will prevent the processing of the application.

9. Bank statement with a minimum balance of K5 million
10. Evidence of compliance with National Pension Scheme Authority and Zambia Revenue Authority
11. Three-year strategic plan
12. Proof of ownership or lease agreement for the training premises
13. Approval from local authority for use of premises (if necessary)
